The Dental Implant Process: Every Step from Consultation to Crown

The full process takes 6-12 months, with most of that time spent waiting for bone to heal. Here is what happens at each stage, what it costs, and when you pay.

Consultation and CT Scan

What happens

Your dentist assesses your mouth, takes a 3D CT scan to map bone density, and creates a treatment plan. This is where you learn whether you need a bone graft, which implant type is appropriate, and what the full cost will be.

What to expect

30-60 minute appointment. No pain. You leave with a written treatment plan and quote.

Payment

Usually due at this visit. Some practices waive the consultation fee if you proceed with treatment.

Extraction and Bone Graft (if needed)

Month 1-2

What happens

If the damaged tooth is still present, it is extracted. If the CT scan showed insufficient bone, a bone graft is placed at the same time. Socket preservation material may be packed into the extraction site to prevent bone loss.

What to expect

30-90 minutes under local anesthetic. Mild discomfort for 2-5 days. Soft diet for 1 week.

Payment

Due on the day of the procedure. Extraction and graft are separate line items.

Implant Post Placement

What happens

A small incision is made in the gum. A precision hole is drilled into the jawbone using surgical guides from the CT scan. The titanium post is screwed into position. The gum is closed over the top.

What to expect

1-2 hours under local anesthetic. Swelling and mild discomfort for 3-5 days. Return to normal activities next day. Soft foods for 1 week.

Payment

Due on surgery day. This is typically the largest single payment.

Osseointegration (Healing Period)

3-6 months after placement

No cost (waiting period)

What happens

The titanium post slowly fuses with your jawbone at a cellular level. Bone cells grow into and around the implant surface, creating a permanent bond as strong as a natural tooth root. You wear a temporary prosthetic if needed.

What to expect

No appointments during this period (maybe 1 check-up). Life continues normally. No restrictions after the first month.

Payment

No payments during this period.

Abutment and Crown Fitting

Month 7-12

What happens

The abutment connector is screwed onto the integrated post. Impressions or a digital scan are taken. A custom crown is made in a dental lab (2-3 weeks). At a final appointment, the crown is cemented onto the abutment.

What to expect

2 appointments, 2-3 weeks apart. No surgery. The abutment placement takes 20 minutes. Crown fitting takes 30 minutes. You leave with a complete, functional tooth.

Payment

Abutment and crown are usually separate bills. Some practices bundle them.

Same-Day Implants: When They Are Possible

"Immediate loading" or "teeth in a day" collapses stages 2-4 into a single appointment. The implant post is placed and a temporary crown or prosthesis is fitted the same day. This is possible when:

Good candidates

  • Good bone density confirmed by CT scan
  • High initial implant stability at placement
  • Non-smoker with good general health
  • Front teeth (lower bite force)

Not suitable when

  • Low bone density or recent bone graft
  • Active smoker or uncontrolled diabetes
  • Heavy clenching or grinding habit
  • Multiple adjacent implants needed

Same-day implants typically cost 10-20% more than the standard two-stage approach because of the additional temporary prosthesis and the specialized surgical planning required.

What Can Go Wrong

Dental implants have a 95%+ success rate, but complications do occur. Being informed helps you make a better decision.

Implant failure (3-5% risk)

The post does not integrate with the bone. It loosens and must be removed. After 3-6 months of healing, placement can usually be reattempted. No additional cost for the replacement post at most practices (check your guarantee).

Infection (2-5% risk)

Infection at the surgical site. Treated with antibiotics. Rarely requires implant removal. Risk is higher in smokers and patients with gum disease.

Nerve damage (less than 1% risk)

Numbness or tingling in the lower lip or tongue. Almost exclusively associated with lower jaw implants near the inferior alveolar nerve. Usually temporary, resolving within weeks to months.

Bone graft failure (5-10% risk when grafted)

The graft material does not integrate. Requires a second grafting procedure. Adds time and cost to the overall treatment.

Recovery Expectations

Pain levels

Most patients report mild to moderate discomfort for 2-5 days after surgery, manageable with over-the-counter painkillers (ibuprofen). Pain after placement is often less than after a tooth extraction.

Return to work

Most patients return to desk work the next day. Physical labor: 2-3 days off recommended. You may have visible swelling and bruising for 3-7 days.

Dietary restrictions

Soft foods for 1-2 weeks after surgery. No chewing on the implant side. After 2 weeks, you can gradually return to normal eating. Avoid very hard or crunchy foods for 4-6 weeks.

Oral care during healing

Gentle brushing around the surgical site after 24-48 hours. Saltwater rinses starting the day after surgery. No vigorous rinsing or spitting for 24 hours. Your dentist will provide specific care instructions.
